Please complete this form and submit it to the Grande Prairie & District Chamber of Commerce. Tell us the name of the business, in what category you feel they are deserving, and briefly why. We will contact the nominated business directly to determine if they accept the nomination and to follow up with them for supporting information. This is not a voting process - no need for multiple nominations for the same business.

GENERAL ELIGIBILITY REQUIREMENTS: 

To be a valid entry in the Grande Prairie Chamber Business Awards of Distinction, a business must: 

• Be operating in the Grande Prairie area and continue to do so at the time of the awards presentation; 
• Have been in operation in the Grande Prairie area for a minimum one fiscal year unless stated otherwise 
• Not have previously won in the same category in the past three years; 
• Not have anyone in their management, on their board, or in their employ serving on the Grande Prairie Chamber Awards of Distinction selection committee for the category in which they’ve been nominated.

Award applicants are not required to be a member of the Grande Prairie & District Chamber of Commerce.

Nominations must be received by the Chamber of Commerce by 4:30 PM on Friday, August 18, 2023. Every nominator will receive a confirmation of receipt. If you do not receive confirmation of your nomination, please contact the Chamber Office at 780-532-5340. 

A selection committee will evaluate all supporting information from the nominated businesses in each award category based on a scoring matrix. Recipients will be announced at our Grande Prairie Chamber Awards of Distinction, to be held on Thursday, October 19, 2023 as part of BDC Small Business Week.
