Please complete this form to SELF-nominate your business and submit it to the Grande Prairie & District Chamber of Commerce. Tell us the name of your business, in what category you feel you are deserving, and provide supporting information based on the category you choose. If you’re not sure what category would be the best fit, nominate your business anyway for being awesome and we will work with you to find the one that suits you best!

Nominations must be received by the Chamber of Commerce by 4:30 PM on Friday, September 24, 2021. Every nomination will receive a confirmation of receipt. If you do not receive confirmation of your nomination, please contact the Chamber Office at 780-532-5340. 

A judging panel will adjudicate all submissions in each award category. All nominees will also be considered for our Small Business of the Year Awards for businesses with under ten employees and with 10-to-50 employees. Recipients will be announced at the Grande Prairie Chamber Awards of Distinction, to be held on Thursday, October 21, 2021 as part of BDC Small Business Week.
