Please complete this form to SELF-nominate your business and submit it to the Grande Prairie & District Chamber of Commerce. Tell us the name of your business, and in what category you feel you are deserving. You will then be contacted to continue the awards process which includes creating a profile and providing supporting information for your nomination. 

Nominations must be received by the Chamber of Commerce by 4:30 PM on Wednesday, August 31, 2022. Every nominator will receive a confirmation of receipt. If you do not receive confirmation of your nomination, please contact the Chamber Office at 780-532-5340. 

A selection committee will evaluate all submissions in each award category. Recipients will be announced at the Grande Prairie Chamber Awards of Distinction, to be held on Thursday, October 20, 2022 as part of BDC Small Business Week.
